These Terms of Use are provided by Templum Aesthetics & Wellness (“Templum,” “we,” “us,” or “our”) and govern your access to and use of our website https://templumaw.com, including appointment booking and communication services such as SMS and email.

By accessing or using our website or booking an appointment, you agree to be bound by these Terms of Use. If you do not agree with these terms, please do not use our services.

1. Use of Our Services

You agree to use our services only for lawful purposes and in accordance with these Terms. You must be at least 13 years old and have the legal authority to book an appointment.

  • We reserve the right to cancel or reschedule appointments due to unforeseen circumstances.

  • Appointment confirmation may be sent via SMS or email if you opt in to receive such communications.

3. SMS Consent

By selecting the checkbox during appointment booking, you consent to receive SMS messages related to your appointment. Message and data rates may apply. You can opt out at any time by contacting us.

4. User Conduct

You agree not to:

5. Intellectual Property

7. Disclaimer & Limitation of Liability

Our services are provided “as is” without warranties of any kind. We are not liable for any damages resulting from your use of our website or services.

8. Gift Card Terms & Conditions

Effective Date: 01st Aug, 2026

These Gift Card Terms & Conditions apply to gift cards purchased from Templum Aesthetics & Wellness (“Templum”) for use at participating Templum Aesthetics & Wellness locations in Washington State.
By purchasing, accepting, or using a Templum gift card, you agree to these Terms & Conditions.

8.1. Gift Card Value

Templum gift cards are issued in U.S. dollars and may be applied toward eligible services and products offered by participating Templum Aesthetics & Wellness locations in Washington State, subject to these Terms & Conditions.

A gift card represents a prepaid monetary value and does not guarantee any particular service, treatment, product, provider, appointment date, or price.

8.2. Digital Delivery

Digital gift cards are delivered to the recipient email address provided by the purchaser.

The purchaser is responsible for verifying that the recipient’s name, email address, delivery date, and other information are correct before completing the purchase.

Templum is not responsible for delivery delays or failures resulting from an incorrect email address, spam filtering, email-provider issues, or other circumstances outside Templum’s reasonable control.

8.3. Scheduled Delivery

When available, purchasers may choose to have a digital gift card delivered immediately or on a selected future date.

Scheduled delivery dates are provided as a convenience and are not guaranteed to occur at a specific time of day.

8.4. Redemption

Gift cards may be redeemed toward eligible purchases at participating Templum Aesthetics & Wellness locations in Washington State.

The gift card number or other requested verification information must be provided at the time of redemption.

Gift cards may be used over multiple transactions until the available balance has been depleted.

If the purchase amount exceeds the available gift card balance, the recipient is responsible for paying the remaining amount using another accepted form of payment.

8.5. Remaining Balances

Any unused balance will remain associated with the gift card and may be applied toward future eligible purchases.

If the remaining balance qualifies for cash redemption under Washington State law, the holder may request that the qualifying balance be redeemed for cash.

8.6. Medical and Aesthetic Treatment Eligibility

Possession of a Templum gift card does not guarantee eligibility for any particular medical, wellness, injectable, laser, aesthetic, or other treatment.

Certain services may require a consultation, medical history review, examination, prescription, provider approval, or other eligibility requirements before treatment.

All medical and aesthetic treatment decisions remain subject to the independent professional judgment of the treating provider.

If a recipient is not eligible for a particular treatment, the gift card balance may be applied toward other eligible Templum services or products, subject to these Terms & Conditions.

8.7. Appointments and Availability

Purchasing or receiving a gift card does not reserve an appointment.

All appointments are subject to provider, treatment, equipment, and scheduling availability.

Recipients are encouraged to schedule appointments in advance.

Standard Templum appointment, cancellation, rescheduling, late-arrival, and no-show policies continue to apply when a gift card is used.

8.8. Service Prices

Gift cards represent a dollar value, not a guaranteed price for a particular treatment unless expressly stated otherwise in writing.

Prices for Templum services and products may change over time. The price in effect at the time a service or product is purchased or provided will generally apply.

For example, a $500 Gift Card provides $500 of purchasing value; it does not lock in the current price of a particular treatment.

8.9. Promotions, Discounts and Special Offers

Unless otherwise expressly stated, gift cards may not be combined with certain promotions, promotional certificates, discounts, membership benefits, packages, or special offers.

Templum reserves the right to establish reasonable restrictions concerning the combination of gift cards with promotional offers.

Promotional, bonus, complimentary, or non-purchased value issued in connection with a promotion may be subject to separate terms and restrictions to the extent permitted by Washington law.

8.10. Expiration

Purchased Templum gift cards will not expire except to the extent otherwise permitted by applicable law.

Promotional or complimentary certificates that are not purchased gift cards may be subject to separate terms where permitted by law. Any applicable expiration date or restriction will be disclosed with the promotion.

8.11. Fees

Templum does not charge inactivity, dormancy, or maintenance fees against the purchased value of a gift card except to the extent expressly permitted by applicable law.

8.12. Refunds and Returns

Gift card purchases are generally final and non-refundable, except where otherwise required by Washington or federal law or expressly approved by Templum.

Services or products purchased using a gift card remain subject to Templum’s applicable refund, return, cancellation, and treatment policies.

Where a refund is approved for a purchase originally paid with a gift card, Templum may return the applicable amount to the original gift card or issue replacement Templum credit rather than cash, except where cash payment is required by applicable law.

8.13. Lost, Stolen or Compromised Gift Cards

Gift cards and gift card numbers should be treated like cash.

Recipients should not publicly share, post, or otherwise disclose their gift card number.

If a gift card is lost, stolen, deleted, or believed to have been compromised, please contact Templum as soon as possible.

Templum may be able to replace or deactivate a gift card if ownership and the remaining balance can be reasonably verified. Replacement is not guaranteed where the gift card value has already been redeemed.

8.14. Unauthorized Use and Fraud

Templum reserves the right to temporarily suspend, cancel, or refuse a gift card transaction when there is a reasonable basis to suspect fraud, unauthorized use, payment reversal, chargeback, technical error, or other unlawful activity.

Gift cards obtained through fraudulent or unauthorized transactions may be canceled to the extent permitted by law.

8.15. Transferability

Unless otherwise restricted, Templum gift cards may generally be given to another person.

Gift cards may not be reproduced, altered, counterfeited, or used for unauthorized promotional or commercial purposes without Templum’s written permission.

8.16. Reloading Gift Cards

If Templum offers gift card reloading, additional funds may be added to an eligible existing gift card through an approved Templum purchasing method.

Reloaded funds will be subject to these Terms & Conditions and applicable Washington law.

Templum may change or discontinue the reload feature, provided that doing so does not unlawfully affect previously purchased value.

8.17. Gift Card Balance

Gift card holders may contact Templum or use an available online balance-checking feature to determine the remaining value of a gift card.

Templum’s records regarding gift card purchases, redemptions, adjustments, and remaining balances will control unless there is a demonstrated error.

8.18. Taxes and Additional Charges

Applicable taxes, fees, or other charges associated with services or products purchased using a gift card remain the responsibility of the recipient and may be deducted from the gift card balance where applicable.

8.19. Cash Redemption — Washington State

Templum gift cards are not generally redeemable for cash except where required by Washington State law.

If the remaining balance on an eligible gift card is less than $5.00 after a purchase, the gift card holder may request that the remaining balance be redeemed in cash in accordance with Washington State law.

Other cash-redemption rights provided by applicable Washington law will also be honored.

8.20. Cancellation or Modification of Gift Card Program

Templum may modify its gift card program, these Terms & Conditions, or available gift card features from time to time.

Any changes will be subject to applicable law and will not unlawfully reduce or eliminate previously purchased gift card value.

8.21. Website and Technical Issues

Templum will make reasonable efforts to maintain accurate gift card balances and reliable electronic delivery and redemption systems.

In the event of a technical or administrative error, Templum reserves the right to investigate and correct the affected transaction or balance.

8.22. Governing Law

These Terms & Conditions are governed by applicable federal law and the laws of the State of Washington, without limiting any consumer rights that cannot legally be waived.

9. Changes to These Terms

We reserve the right to modify these Terms at any time. Updates will be posted on this page with a revised effective date.
